Refer a family to Ability Avenues

For pediatricians, IEP teams, and county staff: we serve children ages 2–12 with ASD or related conditions — center-based in Golden Valley or in-home across most of Minnesota.

Referral criteria

  • Ages: children 2–12.
  • Diagnosis: handled flexibly — we facilitate the CMDE through partner agencies during intake.
  • Funding: Straight MA and BCBS PMAP (TEFRA is a path into MA for other families).
  • Service area: Twin Cities metro for center-based; most of Minnesota for in-home.

Helpful to include: family contact, child's age, diagnosis or evaluation records if available, and coverage information. Missing pieces never block a referral — we gather the rest during intake.

Pediatric & primary care

EIDBI eligibility runs through the CMDE, and we facilitate that evaluation through independent partner agencies during intake — so a referral doesn't need a completed eval attached. Existing diagnosis or evaluation records help us move faster.

Schools & IEP teams

ABA therapy complements school services rather than replacing them — we coordinate with IEP goals and schedules so a child's supports pull in the same direction.

County & case workers

We help families navigate MA and TEFRA paths into coverage and take on the intake paperwork — useful when a family you support needs care coordination more than another form.

  • Do you take referrals for children without a diagnosis?

    Yes. Diagnosis status is handled flexibly — we facilitate the child's CMDE through independent partner agencies as part of intake, and a valid existing evaluation can be reused.

  • Which coverage do you accept?

    Straight MA (fee-for-service Medical Assistance) and Blue Cross Blue Shield PMAP. For families with other coverage, TEFRA and MinnesotaCare can be paths into MA — we counsel families on those options by phone, so an "other" answer on the form is still worth submitting.

  • What's your service area?

    Center-based services at our Golden Valley center for the Twin Cities metro; in-home services across most of Minnesota, confirmed case by case during the first call.

  • How will I hear back after referring?

    The form asks for your preferred contact method — phone, email, or fax — and our intake team follows up with you directly, alongside the family.
